THE PROBLEMS
Situation #1
You are in the doctors office, urgent care, emergency room, or maybe even at your home with the ambulance and EMTs there to help your loved one.
Your brain is trying to cope with and deal with your sick or injured loved one and figuring out the best way to help them and dealing with the fear of the unknown.
Someone is asking you all kinds of questions about their medical history and trying to snap you back into reality, or asking you to fill out forms that they have to have so they can properly take care of your loved one.
You can’t think straight and just want to scream, “SOMEONE HELP ME…I NEED HELP…HELP MY PERSON.” There is nothing left for your brain to gain refocus of the details you so desperately need in that moment.
Situation #2
You are getting ready to go on a trip abroad or camping with a group or a field trip that requires medical forms.
You don’t have a whole lot of time to think of the medical history as you are busy getting all the essentials together and realize you have forgotten more than you can remember and need to have a place where you are keeping it all down.
Situation #3
You are a mom and need to keep track of all your kids and husbands medical information so that you can have access to it at the drop of a hat in case you need to reference it and to keep track of all the changes and be ready at all times.
Situation #4
The point is with this that we could go on and on listing all types of situations, but here is the bottom line.
If you are a human and are breathing, YOU NEED THIS FORM.
Whether you are a business owner and want to be prepared and on the ready should something occur, you are prepared to protect your team members with this one asset, or whatever the case may be. This is the basics of basics and could very well help save your life or the life of a loved one some day.
THE MOMENT OF CLARITY
Caregiving for my completely paralyzed mom, everytime we went to a doctor, we were handed forms to fill out. Meanwhile while we were anxiously tending to mom emptying her colostomy, changing her inner cannula, running out to the van to grab her cough machine if she couldn’t breathe, and the list goes on and on, we were supposed to be simultaneously remembering her entire medical history and her details and filling it out!!
SOMETHING HAD TO GIVE AND IT WASN’T GOING TO BE HER CARE OR SHE WOULDNT HAVE SURVIVED!
So, at the doctors one day in utter tears and exhaustion I went to the nurses station and said…can I fill this out when we get back to the room because if I stop to do this with no one tending to her, she could die. I need help. Is there anyway that if I type up her entire history and keep it updated…could I just bring that in any time we come and not fill out your forms?
She smiled and said absolutely that would work!
I went home that day even though exhausted and while Dad was caring for Mom, I got my computer out and typed up a quick medical history with all her current stats and needs and recent and prior medical history.
Fast forward to a couple of years ago, my husband and I were talking and it dawned on us…EVERY SINGLE ONE OF US NEEDS THAT FORM.
I WILL TELL YOU WHY.
During my years as an EMT, there were so many times when we would run on completely healthy individuals that had been in a car wreck or any number of other things that can happen to anyone and when we were asking the medical history to a nearby loved one, the majority of the time, they could not tell us what we needed to know…why? Because that’s what trauma does to you…your brain is so busy trying to process what is happening that most of us are unable to unlock that other part of our brain where the important information resides.
Fast forward to Mom’s caregiving years when I came up with that form and the anguish and pain it not only saved us mentally during appointments, but was a lifesaver to us and the doctors in the way of constantly updating it and being able to look back on her history and see what works and where we have come or if we needed to know when a surgery was or when meds changed.
As a mother, I know first hand how information for your different kids (or even if you have 1) can get jumbled in your brain and how when you need that information most, it can fail you as you are in the throws of stress and coping.
So, whether you are single living alone or with your pet (your pets need one too!), married, single parent, retired, or whatever station in life you are in and no matter how healthy you are, anything can happen at any time.
Now, don’t get me wrong, this isn’t about worry or fear of the unknown. No, it’s quite the opposite. It’s proving how strong of mind and body you are to be a planner and make a plan and be prepared for anything life throws at you or even just to have in case you need to know for the future.
THE SOLUTION
I came up with this out of sheer desperation and exhaustion during my moms early caregiving years. Don’t wait until you are in the throws of exhaustion and completely overwhelmed.
It is a simple medical form that I have customized so that you can fill out your information on the computer and then print it out and constantly update it and print out new copies as you go.You can also just print it out and fill it in if that’s more your style.
Whatever you do, do yourself and your family a favor and GRAB THIS FREEBIE that will always be free as I believe every single person alive needs this!
Yes, it does take time if you have never done this before to sit down, go back and remember the pieces and fill it in. Don’t be discouraged at all. Keep it up in your screen and as you think of things, jot them down and run to type it in. It will all come together eventually.
Also, please note there is a place on the first page at the top that says date last updated! This is sooooo important! From an EMT perspective, we need to have current information to help them make split second decisions. Also, please remember, this is not a time to hold back. Embarrassment and privacy don’t play a role when it comes to medical care. Let it all spill out here because someone’s life could depend on it.
